Transportation Request Forms - UPDATE

06/22/2020 IMPORTANT UPDATE

Hard copy, Act372 Transportation forms are required IN ADDITION TO the digital form that was sent out to all families. All families must complete both the digital form and this paper form in order to secure transportation for the 2020-2021 school year.

Dear AGCS Parent/Guardians;

Per guidelines from the PA Department of Education, Pennsylvania school districts are required to have a new/updated transportation form on file for all new and returning students every school year. This means that all AGCS families will be required to provide an updated transportation request form for ALL students on a yearly basis.

With the school being closed due to COVID-19, in April, AGCS took each district’s transportation form and made it a digital survey to collect the data quickly in order for districts to begin to build transportation routes for the 2020-2021 school year.

Per the PA School Code, school districts are still required to collect the PAPER, HARD COPY of the Act372 Request for Transportation form from all families for auditing purposes.

Failure to complete this paper form will result in your district suspending your child’s transportation arrangements.

The included forms are required to be completed by every family whether you are requesting or declining transportation services by your district.

If your district requires any type of child-care information for alternative transportation you must also fill out the associated forms.

We are requesting that all families print, complete, sign and return this form to AGCS by July 10, 2020. We will collect, organize and then transfer these forms to your home school district for you.

If you have not completed the digital form you can access your district’s links below. Data collected via these digital forms will be sent to districts to begin to build transportation routes for the beginning of the year. Failure to complete the digital form may result in your district not providing your child with transportation arrangements for the beginning of school.
