Attention all AGCS Parents & Guardians:
Student Directory OPT-OUT & Delivery Form
The PTO is compiles a comprehensive Student Directory annually. The Directory includes your Child's Name, Grade, and Teacher's Name, as well as, Parent/Guardian Name(s), Home Address, Home Telephone, and Email Address.
You need only fill out one form per family. In doing so, please refer to the youngest child in your family attending AGCS. Please include your First and Last names especially if it is different from your child's last name. Also note that True/False answers will constitute a Yes or No.
Please be sure to indicate your Student Directory delivery preference. You may receive the Directory via email or purchase a printed copy.
**Please note: Failure to submit a form will constitute permission to print the student's information in its entirety.**
E-copies = FREE
Printed copy = $5 each
